         Please enter your legal name on the application and your entire middle name. Your name on your name badge at the annual reunions can be anything you want it to be including a nickname. Please do not use nicknames on the application. We must be accurate about the names as some persons have the exact same name as others. Please also indicate if your are a Jr., Sr. or have any other title such as MD or DDS.



IT IS VERY IMPORTANT THAT THE VIEW MODE ON YOUR COMPUTER IS NOT SET TOO HIGH OR THIS APPLICATION FORM WILL PRINT ON 2 OR MORE PAGES. IN ORDER TO LOWER THE VIEW MODE, HOLD DOWN THE "CONTROL" KEY AND HIT THE MINUS SIGN WHICH IS RIGHT OF THE NUMBER ZERO.
